Dr. Goodman graduated from the University of New England College of Osteopathic Medicine in 2012. From there she completed her residency training in Family medicine and then completed a fellowship in neuromusculoskeletal medicine.

How to become a patient

Step 1: Have your Primary Care Physician (PCP) fax a referral to fax: (833) 450-4808

Step 2: Call the office to schedule an appointment. Please leave a message with your preferred times and days of the week for an appointment.

Step 3: Complete the New Patient Forms located on the website. See below

Referrals are required however if you do not have a PCP and are unable to get a referral please call the office to inquire further on how to become a patient without a referral. Please allow 1-2 weeks for a call back.

Please print and complete the appropriate forms and bring with you to your appointment

If you are unable to print the forms ahead of time, a copy will be provided at your visit. However, please review the forms thoroughly ahead of time so that you are prepared to sign them at the beginning of the visit. Your visit will not start until the necessary forms are reviewed and signed.
